Mandatory Forms On-Site
Students must fill out two required OCS forms at the beginning and conclusion of their time away.

At the start of each semester away, students are required to complete the following form:
This form is critical for academic approval of courses and support in case of emergency. This form is the student's responsibility to complete in a timely manner, and ahead of their host institution's add/drop period.
At the end of your time off-campus, you will be required to complete a Post-Program Evaluation, which will be emailed to you. This mandatory evaluation is essential feedback for our office and future OCS students about the academic and non-academic elements of your experience.
